This online registration form allows the Van Cortlandt Titans to electronically gather important information about its players and their families. In addition to this online registration, parents and players must also complete and hand in the paper Titans Signature Packet before the Player may participate. You can download and print out the 2022 Titans Signature Packet from this website by clicking on DOCUMENTS. Please complete every question below and attach the required documents. You cannot submit the online registration without the attachments. Players who will be 14 or younger on December 31 should join the Bantam team. Players who will be 17 or younger on December 31 should join the Intro team. If you turn 18 or older before December 31, you cannot participate. Information collected here is for use by the Van Cortlandt Titans organization only, and will not be shared with any other entity unless compelled by law. Demographic information about race and ethnicity is requested by the City of New York’s Department of Youth and Community Development.

Pictures of all of the following items must be attached in order to complete this registration. - Proof of Date of Birth: Birth Certificate, Passport or Government issued ID. - Latest report card: Your name, the school name and the date must be visible. - Proof of Health Insurance: Front and back of your health insurance card.
